The following insurance benefits are provided to PrudentChoicePlus™ members as added separate benefits at no additional cost to members.
$5,000 Accident Medical/Dental Benefit*
Pays for medical expenses resulting from a covered accident up to the maximum benefit amount per covered incident. Subject to a $100 deductible.
Some covered expenses include:
• Ambulance
• Hospital room and board
• Ancillary hospital expenses including operating room, lab tests and anesthesia
• Registered nurse services for private duty nurse while confined to a hospital
• X-ray expenses
• Prescription drug expenses
• Dental expenses including dental x-ray
$15,000 Accidental Death & Dismemberment*
Accidental Death and Dismemberment (AD&D) provides additional funds in the event of a fatal accident or an accident that result in the loss of a limb or eyesight.
If Your Injury results in any of the following losses within 365 days after the date of the Covered Accident, The amount shown below will be paid for that loss. If multiple losses occur, only one Benefit Amount, the largest, will be paid for all losses due to the same Covered Accident.
