The Individual Membership option provides healthcare discount savings for the active member only. The Family Membership option includes the active member plus any member of your household. The Membership Card will contain the primary cardholder's name only. For Family Memberships, the cards will be valid for all family members.

PrudentChoice™ guarantees full satisfaction with its service. Members may cancel their PrudentChoice™ program at any time and for any reason by requesting cancellation in writing and sending it to Careington International at 7400 Gaylord Parkway, Frisco, TX 75034. If a Member cancels within the first 45 days of the activation date, the Member will receive a refund of the first month’s membership fee. If a Member cancels after the first month, the Member will no longer be billed subsequent membership fees. No refunds of the one-time processing fee will be made.
